Step 1 - Fill out this short Apparel Order Form

This form will help us design the perfect apparel for you!
Please answer these few questions to the best of your ability.
Communication is the key to this whole thing. The better you communicate to us the better and quicker your apparel will be.

Minimum order of 30 shirts please
There will be a one time cost to design shirt.

$1 extra for printing on the back.

Step 3 - Shipping & other stuff

• Gildan 100% cotton 6.1oz
• Price includes (S)Small, (M)Medium,
(L)Large, & (XL)Extra Large
for 2X add $1, for 3X add $2
for 4X & 5X add $3.
printing on sleeve available for
$2.00 extra per shirt
oversized printing (larger than 12 inches)
available for $2.00 extra per shirt
• Underlay is $1 extra.

Step 3 - Shipping & other stuff

Shipping is measured in business days.

30-100 - $8.00ea
101-200 - $7.75ea
201-300 - $7.50ea
301-400 - $7.25ea
401-500 - $7.00ea

*When you give me all this information I will do a proof for you and will send it via e-mail. Typical turnaround time on getting the proof to you is about 48 hours. Once you check for typo's and approve the proof I will begin the screening process. I will send the invoice via e-mail in PDF format. You can pay by check or Paypal.

*Orders will be shipped to the address you provide within two weeks from the date of proof approval. I ship UPS ground and Shipping is FREE. Next day air shipping is extra. Rush orders please call for price.

Important: Once we design the shirt and do a proof it is assumed that the client agrees to the price and is committed to ordering the shirts. Please don't let us design a shirt for you only for you to call back saying you don't want them anymore. We spend a lot of time and energy in making sure you have an awesome design. Thanks for your understanding.
